The role

As a Community Care Coordinator, you will work closely with the care management team to ensure care team members are providing high quality care to clients within a set geographic across community services. 

Having an engaging & collaborative communication style you will easily build relationships & links within the community with your clients, staff & wider networks ensuring a personalised service is delivered to each client.

Holding an excellent understanding of industry standards & clinical indicators you will ensure that all staff are educated & supported in delivering care needs, with the ability to highlight any areas of improvements.  

Summary of Duties

• Provide clinical guidance to staff surrounding client care
• Undertake & review client assessments
• Ensure accurate completion of relevant documentation
• Motivation, training & development of team members
• Ensure compliance & quality indicators are met
• Ensure funding models are met & updated

 

Skills and Experience

• AHPRA registration within nursing
• Previous experience within a leadership role
• Previous experience with HCP funding & care planning
• Experience supporting clients within the community
• Understanding & working knowledge industry standards
• Ability to identify & implement areas of improvement
